Diabetes Pathways Project

When to see a CDE – advice for people with diabetes at different ages and stages of life:

A report on the background and development of Diabetes Referral Pathways

There was previously no clear, Australian Diabetes Educators Association (ADEA)-endorsed Australian information for people with diabetes, healthcare providers and others that sets the standard for when people with type 1, type 2, gestational or other types of diabetes, should see a Credentialled Diabetes Educator (CDE) for diabetes education, care and management.

ADEA have developed Diabetes Referral Pathways that provide guidance in the form of diagrams, summarising evidence and expert consensus to develop ‘gold standard’ pathways for diabetes education throughout the diabetes journey from initial diagnosis through to follow-up/review consultations.

This report provides the context and background into the development of the Diabetes Referral Pathways.
