• Understand what is diabetes & how to self-care at home.
• Set-up with glucose meter & teach to monitor blood glucose.
• Register with NDSS to access subsidised supplies.
• Managing hypoglycaemia & hyperglycaemia.
• Support you to start on glucose lowering medications.
• Support you to start injections.
• Set-up with continuous glucose monitoring (CGM) system.
• Manage your blood glucose when unwell.
• Travel and diabetes.
