About the role
The role of the Diabetes Nurse Educator (DNE) is to provide direct clinical care and intervention according to the scope of practice of their primary health discipline. The role will also require liaising closely with external and internal stakeholders and promoting the DNE service. This role will be based at Kensington but may require service provision at other sites.

The Bathurst Diabetes Service offers holistic diabetes care in all aspects of diabetes management: GDM, T1DM, T2DM, paediatrics to adults, inpatients and outpatients. It is located within the Community Health Centre at Bathurst Hospital enabling the Clinical Diabetes Educator (CDE) to liaise closely with other health care disciplines including dietitians and integrated care in coordinating referrals and management.
There is a visiting Endocrinologist consulting at the hospital on a weekly basis as well as a new part time private Endocrinologist consulting in the community. The CDE also has a responsibility in the provision of education for medical and nursing staff and is a great opportunity to implement your clinical expertise in diabetes management. Familiarity with the use of the latest technology in diabetes management is essential.

Position
This is a new Diabetes Nurse Practitioner (NP) role within the recently established dedicated Endocrine & Diabetes Type 1 Service. The NPs primary focus is to improve the health outcomes of people with type 1 diabetes, especially those using technology including insulin pumps and continuous glucose monitoring. The Diabetes NP role is intricately designed to address the dynamic demands faced by the Endocrine & Diabetes service, operating both autonomously and within a multidisciplinary framework. The Diabetes NP is responsible for the provision of advanced clinical practice and is empowered to provide timely, high-quality care, involving advanced clinical assessments, medication prescription, diagnostic orders, ongoing treatment, and the capability to refer patients to specialists. This practice is firmly rooted in evidence-based principles, utilizing resources such as Alfred Health clinical guidelines and expert-endorsed practice guidelines. Operating within both autonomous and collaborative care models, the Diabetes NP takes professional responsibility for the entire patient care episode in autonomous scenarios and consults senior medical colleagues in collaborative decision-making. The degree of autonomy is carefully determined by factors such as patient age, clinical complexity, and co-morbidity. In cases without significant clinical complexity, the Diabetes NP is authorised to autonomously assess, diagnose, and treat patients.

Barwon Health’s Nutrition & Dietetics Department provides services across the care continuum including Acute Care based at University Hospital Geelong, and Rehabilitation, Subacute and Aged Care based at McKellar Centre. Our team consists of 20 FTE Dietitians and 2.4 FTE Dietitian Assistants across both sites. We provide dietetic services to inpatients and outpatients across all clinical specialties according to current best practice and professional standards. We work collaboratively across sites, as well as with our community-based colleagues. Our staff pursue clinical excellence, consumer-centred care and interdisciplinary teamwork. Position
This is a new Diabetes Nurse Practitioner (NP) role within the recently established dedicated Endocrine & Diabetes Type 1 Service. The NPs primary focus is to improve the health outcomes of people with type 1 diabetes, especially those using technology including insulin pumps and continuous glucose monitoring. The Diabetes NP role is intricately designed to address the dynamic demands faced by the Endocrine & Diabetes service, operating both autonomously and within a multidisciplinary framework. The Diabetes NP is responsible for the provision of advanced clinical practice and is empowered to provide timely, high-quality care, involving advanced clinical assessments, medication prescription, diagnostic orders, ongoing treatment, and the capability to refer patients to specialists. This practice is firmly rooted in evidence-based principles, utilizing resources such as Alfred Health clinical guidelines and expert-endorsed practice guidelines. Operating within both autonomous and collaborative care models, the Diabetes NP takes professional responsibility for the entire patient care episode in autonomous scenarios and consults senior medical colleagues in collaborative decision-making. The degree of autonomy is carefully determined by factors such as patient age, clinical complexity, and co-morbidity. Barwon Health’s Nutrition & Dietetics Department provides services across the care continuum including Acute Care based at University Hospital Geelong, and Rehabilitation, Subacute and Aged Care based at McKellar Centre. Our team consists of 20 FTE Dietitians and 2.4 FTE Dietitian Assistants across both sites. We provide dietetic services to inpatients and outpatients across all clinical specialties according to current best practice and professional standards. The Diabetes Nurse Practitioner (NP) is an expert practitioner who, through leadership, will facilitate research-based clinical practice aimed at providing health care to patients with complex needs within a defined scope of practice. The primary responsibilities of the Diabetes NP will include advanced clinical assessment using diagnostic capability, planning and engaging others, prescribing and implementing therapeutic interventions and evaluation of outcomes to improve practice.
The Diabetes NP will provide assessment and management using advanced nursing knowledge and skills. The Diabetes NP will have extensive experience in Diabetes management and will provide clinical and professional leadership within the community environment. The Diabetes NP will conduct independent comprehensive patient assessments and manage a range of Diabetes related patient conditions within the community setting including residents of Residential Aged Care Facilities (RACF’s) and Homeless Health environments.
The Diabetes NP will contribute to the advancement of quality health care systems within the division of the community setting to reduce avoidable hospital admission and promote early discharge from hospital. The Diabetes NP will support the continuum across the inpatient and community setting and is an expert on service availability promoting the safe discharge planning and ongoing support for the Diabetic patient.
The Diabetes NP will develop a sustainable model of care in collaboration and in partnership with general practitioners, specialist medical officers, RACF’s, Community Health Services and other health professionals. The intervention will be in conjunction with the patient and/or family and will develop person-centred plans of care specific to the patient’s complex needs.
